ATTENTION: During your initial registration you will be asked to provide all documents listed below. We do apologize for the inconvenience however we are striving to make this transition as smooth as possible for you as well as our future students.

  • Birth Certificate

  • Social Security Card

  • Ga Form 3231 Immunizations

  • Ga Form 3300 Ear, Eye, Dental Exam

  • 2 Proofs of Residence

  • Residency Affidavit 

  • Transcript (can be unofficial) or a withdrawal form with current class grades

***Please notify the Registrar immediately if you have Guardianship and/or Power of Attorney for the student you are registering.

Supporting documents must be provided at the time of registration, otherwise, your packet will not be processed until this can be provided or you are able to provide documentation from the courts that the process has been started.

***Please notify the Registrar immediately if your child receives any of the following services:

  • Special Education

  • 504 Accommodation

  • Gifted and Talented

  • ELL (ESOL)
